Eligibility and application process

Before submitting an application for transport the school must assess that the student is eligible to apply and meets the eligibility criteria.

Step 1: Assess eligibility

When working with a parent the school needs to check the eligibility criteria to ensure that the student and the parent meet the criteria.

The assessment of travel support needs is provided to assist schools in the assessment of the individual student travel support needs.

Step 2: Download and complete forms

Schools will work with parents to complete Part A and then the school will complete Part B.

Application for Assisted School Travel overview

  • Page one. The cover page includes an introduction to the ASTP and instructions to complete the application form.
  • Pages 2 to 4 are for Part A, to be completed by the student’s parents/carer and then returned to the school principal.
  • Pages 5 to 8 are for Part B, to be completed by the school principal and forwarded with Part A to the ASTP for assessment.

How to complete Part A

  • All fields marked with a red asterisk must be completed for this application to be accepted for review.

School Details.

  • Enter the name of the school the student is applying for transport.

Student Details.

  • Enter the student's first name, other names if relevant and last name.
  • Enter their date of birth and gender.
  • Tick the box that applies for identifying as Aboriginal and/or Torres Strait Islander.
  • Enter the student's full residential address.

Transport Details.

  • If the transport address is different to the residential address, complete this section if relevant.
  • Enter the estimated travel distance in kilometres between the student's home and school.
  • Tick the box that applies if this student travels independently outside of school hours.
  • Tick the box that applies if this student has shared care arrangements.
  • If yes, please attach a copy of the shared care form available from the ASTP website.
  • Enter the start and end dates for the transport period you are requesting.
  • Tick the days you are requesting AM transport, and enter the adult supervisor who will be present when the student is getting into the vehicle.
  • Tick the days you are requesting PM transport, and enter the adult supervisor who will be present when the student is getting out of the vehicle.
  • Tick the box that applies if other school-aged children in your care, are enrolled at a school.
  • If yes, enter the student's name, age, school name and bell times, and how they travel to school.

Parent/Carer Details.

  • Enter the primary contact details.
  • Enter their title, first name, last name, and relation to the student.
  • If their residential address is different to the student's address, complete this section if relevant.
  • Enter a home phone number if available, mobile number, and email address if you have one.
  • Enter the reasons why you are unable to provide transport for the student.

Emergency Contacts.

  • Enter an emergency contact that is not a person already listed in this application. Enter their full name, and relevant phone numbers.

Work Details.

  • Tick the box that applies if you work, that would prevent you from providing or arranging transport for the student to travel to and from school.
  • If yes, enter the name of your employer, address, phone number, and the days and times you work.

Medical Details.

  • Tick the box that applies if you have a medical condition or carer responsibilities, that would prevent you supporting the student's travel to and from school.
  • If yes, please arrange for your medical practitioner to complete the departments medical certificate (available on the ASTP website) and attach it to this application.

Education Details.

  • Tick the box that applies if you attend TAFE or university, that would prevent you from providing or arranging transport for the student to travel to and from school.
  • If yes, please attach a copy of your timetable to this application, and complete the following details.
  • Enter the name of the education facility, address, and phone number.
  • Tick the days you attend, and enter the times.

Declaration.

  • Carefully read the list of declarations, and tick all the boxes to confirm you understand and acknowledge them before submitting this application.
  • Sign and date.
  • Please ensure that all additional required documents are attached to this application, and tick the boxes that apply.
  • Return the completed Part A to the school principal, either by email, post or in person.
  • Pages 5 to 8 are for Part B, to be completed by the school principal and forwarded with Part A to the ASTP for assessment.

Documentation checklist.

  • Please ensure the listed documentation (as applicable) is attached to Part A and B before submitting to the ASTP.

School Details.

  • Enter the name of the school address, phone number, email, and the school principal’s name.
  • Enter the school start and finish times and the relevant person who will be the contact for the ASTP.

Student Details.

  • Enter the student's first name other names if relevant and last name, then their student registration number (SRN).
  • Tick the boxes that apply to the student's disability.
  • Tick the box for the student's class type.
  • Enter their enrolment level - grade at school.
  • An assessment of the individual travel support needs of the student must be undertaken by the school in accordance with the Assessment of Travel Support Needs - Guide for Schools, available on our website.
  • Tick the box that applies from the result of the assessment.
  • Please note: Ticking Level 1 indicates student could travel independently on public transport or a school bus after participation in an appropriate travel training program.
  • For further information, email the education team at the ASTP.

Work, Health and Safety.

  • Tick the boxes that apply for WH&S considerations related to the student’s assessment, including any specific details and attach additional information as necessary.

Attending School Part-time.

  • If a student is not attending school on a full-time basis attach the certificate of exemption from attendance at school form available on the website.

Principal Recommendation.

  • Tick the box that applies.

Principal Certification.

  • Carefully read and tick all the boxes to certify the application is suitable for submission.
  • Enter your name, sign and date.

Submit Application.

  • Email or post both the completed parts A and B to the ASTP for assessment.

Step 3: Provide supporting documentation

Applications forwarded from the school that do not have all of the required supporting documentation will be returned and processing of the application will be delayed.

Please ensure that you are providing the following supporting documentation when forwarding the application to ASTP:

  • formal advice of student placement (government schools only)
  • disability confirmation sheet
  • medical certificates, parent TAFE or university timetable (where applicable)
  • Shared care request form (Where applicable)
  • risk management, behaviour/health care plans (where applicable)
  • other relevant information.

Step 4: Submit application to ASTP

Once the Application for Assisted School Travel form (PDF 564KB) is complete and all supporting documentation has been provided, scan and email application to generalenquiries.astp@det.nsw.edu.au.

Where possible ASTP will assess each application within ten (10) working days.

2024 Applications for Assisted School Travel

  • 13 September 2024 is the last day that the ASTP will accept student applications and variations for 2024 (excludes changes to permanent residential addresses and contact details).
  • 15 November 2024 is the last date the ASTP will make changes to permanent residential addresses and contact details for the 2024 school year.

2025 Applications for Assisted School Travel

  • 23 July 2024 is the first day that the ASTP will accept new student applications for 2025.
  • 18 October 2024 is the last day that the ASTP can guarantee that 2025 student applications will be assessed in time and school transport arrangements made for the 2025 school year.
