The information requested on this form is being collected by the NSW Department of Education (department), located at 105 Phillip St, Parramatta, NSW. The department will use the information collected for the following purposes:

1. To assess whether it is appropriate to provide the applicant access to the data asset; and
2. To create and maintain records about requests to access the department’s data assets for the purpose for governance and auditing; and
3. For the preparation of data sharing documentation.

The department reserves the right to share the collected information with data custodians and governance groups for governance and reporting purposes. If the data requested is held by a third party, the department may need to share the information with that third party to enable them to evaluate the request. The information you provide may also be included in any data sharing agreement that may document the sharing of data.

Provision of this information is voluntary, however, if you do not provide all or any of the information requested, we may not be able to process your request.

You have the right to access and correct the information you provide. If you want to amend your own personal or health information you can complete and send a privacy application for amendment form to data.services@det.nsw.edu.au.
