The information requested on this form is being collected by the NSW Department of Education (department). The department will use the information collected for the following purposes:

1. To assess whether it is appropriate to provide the applicant access to the data asset; and
2. To create and maintain records about requests to access the Department’s data assets for the purpose for governance and auditing.

Provision of this information is voluntary, however, if you do not provide all or any of the information requested access to the requested data asset may be denied.

The department reserves the right to share information including your name and project role with data custodians and governance groups for governance and reporting purposes.

You have the right to access and correct the information you provide. If you want to amend your own personal or health information you can complete and send a privacy application for amendment form to data.services@det.nsw.edu.au.
