Schools can engage the preferred provider using the Scheme Order Form. Staff can reference or attach the brief to the order form as a way of specifying the agreed services. All fields in the Order Form must be completed. When completing the Order Form, please tick Specialist Allied Health and Behaviour Support Provider Scheme’ at the top of the form.
Note that the Order Form is part of a more detailed agreement, the Human Services Agreement, which each provider on the Scheme has entered into. The Order Form ensures schools are protected by the terms of the full agreement.
