This form is used by contractors in advising the ASTP of changes in vehicle details on runs. Only vehicles on runs are required to be updated.

This form is used by contractors and drivers to notify the Assisted School Travel Program (ASTP) of incidents that have occurred during service.
Driver application checklist and declaration form (PDF 258KB)
For contractors to put forth suitably qualified driver(s), including relief driver to provide transport services for the ASTP.
Expression of interest and change request form for Eligible Service Providers (PDF 416KB)
For contractors who are on the current list of eligible service providers, use this form to apply to increase capacity to be considered for in the allocation of transport runs.

This form is to be completed by contractors to enable the department to make payments to their financial institution.
